Coordination

Community Psychiatric Supportive Treatment or Case Management provide individualized supports or care coordination of healthcare, behavioral healthcare, and non-healthcare activities. This service is for adults and youth.

Goal-Oriented

Case Management services, are goal-oriented and individualized supports that focus on improving self-sufficiency through assessment, planning, linkage, advocacy, coordination and monitoring.  

Access

These services are typically provided in the client's natural environments and include those activities designed to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs.

Case Management Services include:
  • Ongoing assessment of unmet needs
  • Assistance in achieving personal independence in managing basic needs and achieving goals as identified by the individual and/or parent or guardian
  • Restoration, rehabilitation, and support of the development of daily living skills needed to function in a natural community environment
  • Assistance with accessing transportation, safe housing, and employment or meaningful activities
  • Assisting substance use clients with getting to and in treatment facilities and reconnecting to community services
