Care Coordination is a service that exists in connection with any other offered service from our agency. We understand that dealing with systems of care (mental health agencies, children's services, juvenile court, and developmental disabilities) can be intimidating. We are here to help you and ake helping you and/or your child a true team effort.

You are the expert on the life that you live and we are here to assist you with getting the care and attention you need.

• Mental Health Care

We are experienced with the mental health agencies in the surrounding area and can assist you with navigating the mental health system to get the best care. We offer CPST which is part of the mental health system of care and we offer excellent services.

• IEP (Individualized Education Plan)

We understand that sitting through IEP meetings can be intense. We also understand that you may not understand the basic structure of the IEP. We are here to sit in the meetings with you and assist with your understanding and help you to form meaningful questions to ask.

• Resources

We are here to help you find and gain access to community resources and assist you with developing natural resources to assist you when you are no longer involved with agencies. The overall goal is to help you and your family get to a point where you can maintain stability and elevate yourselves to a higher level of well-being.
